Birth Certificate FAQS

- Birth Certificates must be official. We only need a copy.

- They can be requested in the county in which your child was born. 

- The certificate of birth from the hospital is not considered official. 

- Children enrolled in the program are required to turn in an official    birth certificate within 90 days of enrollment.
